A Durable Power of Attorney for Health Care is a document in which you formally select an Agent to carry out health care decisions made in advance by you in the event you are unable to do so due to incapacity.  Specifically, you decide whether or not you want your agent continue or discontinue life-sustaining treatment and under what circumstances. Your Agent has the duty to make decisions in accordance with your wishes.  If your wishes are not clearly expressed, your Agent must act in your best interest and in accordance with accepted medical practices.
